We opened Evolve Physical Therapy in 2017, soon after a national corporate chain acquired the practice where we were working in management.

We made this choice to create a patient-centered environment that supported our mission:

Matt Weissbach | Co-Founder, PT, DPT

Matt Weissbach is originally from Canada, and graduated from Queen’s University (2002) and the University of St. Augustine (2017). He has over 20 years of working as a physical therapist, coach, teacher, and consultant.  Currently, he is co-owner of Evolve Physical Therapy. His areas of clinical interest are pre and post-operative rehabilitation, aquatic rehabilitation, and return-to-play testing. He is passionate about working with runners, jumpers, and overhead athletes to improve their performance and keep them competing by addressing biomechanical inefficiencies and dysfunctions. Matt is a United States of America Track and Field (USATF) Level 1 Coach and has spoken at track and field coaches clinics, universities, and international conferences on running, footwear, long-term athlete development, and ACL rehabilitation.

Melisa Abesa | PT, DPT

Melisa is an Oregon transplant, born and raised in sunny southern California. She received her undergraduate education at Eastern Oregon University, where she competed on the NAIA track team in sprints and relays. She returned to her California roots to attend the University of Southern California for her Doctorate of Physical Therapy. She has spent the past five years working in an orthopedic and sports setting; she has treated a very diverse population and variety of conditions. Melisa has a special interest in runners, headache treatments, post-operative care, and adolescent athletes. She defines her treatment style as a collaborative experience incorporating hands on and active therapies to help reach patient goals. She is a huge advocate for patient empowerment, individualized care, and a holistic approach to health and wellness.

Physical Therapist Josh Guyer

Josh Guyer | PT, DPT

Josh grew up in Central Oregon and graduated from the University of Portland in 2013 with a degree in Biology. While at the University of Portland, Josh interned with the athletic department assisting with on-field game coverage for basketball, soccer, and tennis. He graduated from the University of Mary with his Doctorate of Physical Therapy in 2019. His professional interests extend to tennis and golf with over 10 years of experience in both sports, winning an Oregon high school tennis doubles state championship. Josh has two years of clinical experience, previously working in an orthopedic and sports setting, treating a variety of patients. He has interests in orthopedics and sports medicine, treating athletes of all ages, prevention and wellness, and post-surgical rehabilitation. Josh has a strong commitment to promoting health and wellness and tailors his care to meet each patient’s needs and goals.

Physical Therapist Josh Pate

Josh 'JP' Pate | PT, DPT

Josh “JP” Pate is originally from Lafayette, Louisiana. He studied Kinesiology at LSU and graduated with a bachelor’s degree in 2014. After college, JP had his own personal training practice in New Orleans, LA. His professional education continued with the University of St. Augustine in San Diego, CA. In 2019, JP graduated with a Doctorate in Physical Therapy at the top of his class with an award for orthopedics and manual therapy. Following PT school, JP moved to Portland, OR where he practiced in an outpatient orthopedic and sports setting with experience treating various populations and conditions. He has a special interest in treating post-surgical cases, sport-related injuries, injury prevention, holistic health, and wellness. JP emphasizes functional movements, strength training, manual therapy, proactive symptom management, patient education, and empowerment. In his free time, JP enjoys an active lifestyle outside walking local trails with his dog, hiking, paddle boarding, camping, biking, and playing golf.

Matt Whitaker | Co-Founder, MPT

Originally from the East Coast, Matt Whitaker received his professional degree in 1998 from Loma Linda University. He lived in Utah and California prior to moving to the Northwest in 2016. His first 9 years of practice was in neurologic rehab with a focus on vestibular and balance dysfunctions which he continues to treat . Over the past 13 years his area of care has concentrated on orthopedics with a special interest in treating spine dysfunctions. As a distance runner and having completed two Ironman triathlons, he understands multi-sport athlete goals. He is a certified Level 1 Bike Fitter. Outside of treating patients, Matt has taught physical therapy students in the clinic and the classroom. He has lectured to medical students, provided post-graduate continuing education training to physical therapists, directed therapy programs for hospitals and owned a private practice clinic prior to opening and co-owning Evolve.

Jessica Moore | PT, DPT

Jessica grew up in Oregon and has called the west side of Portland home. She was a competitive softball pitcher for 15 years and graduated from Sunset High School.  She obtained both her undergraduate education and Doctor of Physical Therapy degree at Pacific University. For the past 2 years, she has worked in the orthopedic and sports rehab setting where she has experience treating vestibular, neurological, chronic pain, aquatic, acute, post-surgical, and sports-related injuries. Her treatment style combines hands on manual therapy techniques and functional movement exercises. She specifically collaborates with patients to help them reach their goals. She strives to provide the highest quality care to all of her patients and is passionate about making therapy a positive, fun, and engaging experience.

Physical Therapist Jenny Hwang

Yu-Jen 'Jenny' Hwang | PT, DPT

Yu-Jen “Jenny” Hwang PT, DPT was born and raised in Malawi, and completed her Bachelors in Physical Therapy at National Yang-Ming University in Taiwan. She then earned her Doctorate of Physical Therapy at New York University. Jenny moved to Portland, Oregon in March 2021 after practicing in New York City for 3 years in a multidisciplinary setting. Jenny uses many concepts of Pilates for rehabilitation as well as a combination of manual therapy and therapeutic exercises to help clients achieve their personal goals and move better. She is a strong believer of treating the body as a whole and is SFMA (Selective Functional Movement Assessment) certified and Balanced Body® Pilates Instructor trained. She enjoys helping people reach their full potential, no matter what fitness level or stage of life they are in. Jenny loves the outdoors and is exploring everything that the PNW has to offer. Jenny is fluent in English, Mandarin Chinese, and Taiwanese.

Brittany Abeyta
Patient Care Coordinator

Brittany was born and raised in Oregon, and graduated from Sherwood High School. She has years of experience in medical office management and billing. Brittany worked in a chiropractic office for more than 7 years before joining our clinic. Brittany is committed to making patients feel welcome and well informed with their treatment plan, doctor and patient coordination, patient scheduling, and insurance plans.

Fallie Calder
Patient Care Coordinator

Fallie grew up in Eastern Oregon and earned her BA from the University of Oregon (U of O). After graduating, she spent five years in Washington DC, working for Senator Mark O. Hatfield and the US Senate Committee on Banking, Housing and Urban Affairs in Washington DC. Returning to Oregon, she worked as a lobbyist for the Oregon School Boards Association. She then worked as the Director of a private school in Lake Oswego.

She currently lives in NW Portland with her husband and is the mom of three adult children. She spends as much time as possible in Bend skiing and loves anything that gets her outdoors; boating, kayaking, hiking, and mountain biking.

Fallie is a former patient at Evolve Physical Therapy. She enjoys meeting new people and helping patients coordinate their care at our Portland location.
