Research shows that NMES—when used correctly—can significantly improve early quadriceps strength after ACL surgery.

Getting the Setup Right: Comfort and Efficiency Matter
A 2022 study by Flodin and colleagues examined the effects of electrode size and placement on NMES for the quadriceps. Their key finding? Using 5 × 5 cm or 5 x 9 cm electrodes in a long-transverse placement (across the muscle belly, one on the upper lateral thigh and one on the lower medial thigh) produced:
- The best comfort ratings
- Strong contractions with a lower required current
- A setup that’s easy to replicate at home

- For early post-ACL rehabilitation, we utilize true NMES protocols, focusing on strong, isolated quadriceps contractions, to jump-start the recovery process.
Our Phase One NMES Protocol
At Evolve Physical Therapy, your early-phase NMES program is adapted from:
- Electrode placement: 5 × 5 cm or 5 x 9 cm pads in long-transverse orientation
- Frequency: 35–50 Hz
- Pulse duration: 200–400 µs
- Duty cycle: 10–15 s contraction / 50 s rest
- Intensity: Increased until a strong, visible contraction is achieved
- Combination with voluntary effort: You’ll try to contract your quadriceps along with the NMES for maximal benefit
We start in the clinic, then—if appropriate—train you to use NMES at home to maintain progress between visits.
