
Understanding the tissue healing model is vital to rehab. If you don’t understand the healing process, then you can get stuck in it and have chronic pain. Knowledge of this process can differentiate between a good or bad rehab practitioner. There are four phases of healing, and they overlap. The phases are hemostasis, inflammation, proliferation, and maturation. Each phase plays a crucial role in restoring tissue function and strength, and understanding them helps guide appropriate interventions at the right time. By aligning treatment with the specific phase of healing, you can optimize recovery and minimize the risk of setbacks.
Phase 1: Hemostasis (Stopping the Bleeding)
The first phase, known as hemostasis, is immediate and very short-acting. The blood vessels will constrict to inhibit blood flow and blood loss. Blood platelets come in and form clots to stop further injury from occurring. These clots, or scabs, stop more blood from escaping and protect the wound as it begins to heal. The formation of blood clots triggers the start of the second phase. During this phase, the body works to stabilize the injury site and prevent excessive damage. Although brief, hemostasis sets the stage for tissue healing by creating a foundation for tissue repair.

Phase 2: Inflammation (Cleaning Up the Wound)
The second phase is the acute or inflammatory, usually lasting about two to three days. Right after the clot forms, the body sends healing cells to the area. White blood cells and other essential substances help fight germs and remove dead or damaged cells. During the acute phase, inflammatory cells with different functions come into the area and help clean up. The cardinal signs of the inflammatory phase are marked by heat, redness, swelling, and pain. Pain results from the presence of noxious inflammatory chemicals and heightened mechanical sensitivity. At this stage, the pain is usually characterized by aching at rest, tenderness, and heightened sensitivity to touch and movement. Pain is often constant. Even though inflammation might seem uncomfortable, it is an integral part of healing. It only becomes a problem if it lasts too long.
During this phase, the best course of treatment is to use the R.I.C.E method, which stands for rest, ice, compression, and elevation. A component of the rest is very important and means to stop doing what caused the injury in the first place. Rest seems obvious, especially if it was a traumatic injury. Still, the injury is sometimes due to repetitive movement or bad posture over time. Finding the source of what caused the pain is a key to success. Ice causes the blood vessels to constrict and limits the amount of fluid going to the injury. Compression helps to squeeze fluid out of the area, keeps blood from pooling, and prevents further fluid from getting to the area. Elevation is often overlooked. Elevation uses gravity to help bring fluid and blood back to the heart and can prevent further swelling. Thus, elevating the injured body part above the heart is crucial. And if you think about how gravity works, the higher above the heart, the better. With all that said, there is a debate on how much we want to limit the naturally occurring inflammatory response.
Phase 3: Proliferation (Rebuilding the Skin)
The third phase is known as the proliferation phase and usually lasts about three weeks. The scar tissue is laid down during this phase to help repair the damaged tissue. In addition to new tissue being laid down, new blood vessels and nerve pathways will start to regrow. Special cells make collagen, a protein that helps form new tissue. The wound begins to shrink as these new tissues grow. The tiny blood vessels also form to bring oxygen and nutrients to the healing skin.
If the healing goes well, the new tissue will look pink or red and bumpy. Normal healthy tissue has a linear fiber arrangement, but when scar tissue is laid down haphazardly. By adding gentle stress to the newly formed tissue, the fibers will help to align correctly. Understanding how much stress to apply is vital and comes with experience. If too much stress is added, then it can cause damage to the new tissue and restart the healing process.

Phase 4: Remodeling or Maturation (Strengthening the Skin)
The fourth and final stage of tissue healing is the remodeling or maturation phase. This is where weak, immature, and disorganized scar tissue needs to be optimized to get as close to being like the original non-damaged tissue. It is marked by progressive loading to increase the tensile strength and fiber alignment. This phase can last months and even over a year.
Many factors can affect the rate of the healing tissue, including the type of tissue (ligament, muscle, tendon, bone, cartilage, etc.), the person’s age, smoking, diabetes, and other health issues the person may have. Incomplete repairs can cause increased scar tissue formation, which is poorly aligned and leads to tendinosis or other chronic pain.
An analogy I sometimes explain to my patients is to think of tissue healing like a natural disaster. Imagine a town that is destroyed by a fire or a tornado. The inflammatory phase would be when the clean-up crew arrives and starts getting rid of all the debris and garbage of everything destroyed. Then, you have to plan and begin rebuilding the homes and buildings in the town. Rebuilding would be the proliferation phase. Finally, the maturation phase is when the homes and buildings are fully built and functioning again, businesses are restored, the economy is improving, and the community is thriving.
